Background

In the UK, nearly 10,000 people die each year from cancers of the oesophagus and stomach and this number is predicted to rise. The Imperial College London VOC Laboratory has developed an oesophagogastric cancer detection model based on VOCs in baseline exhaled breath (AU-ROC 0.92). This study aims to understand 'where, why and how' these compounds are produced by specifically interrogating the reprogrammed tumour lipidome, since many of the terminal VOCs identified in breath are downstream products of lipid breakdown.

Aims

  1. Evaluate the origins of oesophagogastric cancer VOCs by interrogating the reprogrammed tumour lipidome
  2. Augment the production of diagnostic VOCs by targeting their pathways of production

Methods

Prospective, single centre, interventional study. 100 patients recruited at 1:1 ratio: (i) confirmed oesophageal/GEJ adenocarcinoma and (ii) benign conditions/normal GI tract. Breath samples collected using standardised procedure on TD tubes and analysed using mid-polar, polar and 2D GC-MS-TOF. Includes in vitro and ex vivo experiments investigating the kinetics of VOC production from lipids.
